At Perspectives Psychological Services we are committed to protecting your personal information. We are fully compliant with the current General Data Protection Regulation (GDPR) guidelines, and we are registered with the Information Commissioners Office (ICO).

Access to your personal data

The ways we may have access to your personal data include, but may not be limited to, the following:

  • Information you provide through the ‘Contact us’ form on our website
  • Information you send in emails
  • Information discussed face-to-face, in a session, via Skype, or over the phone.
  • Information provided to us by a referrer (such as a GP or insurance company).
  • Information provided to us by someone else (such as an employer, relative, court).

Information stored by us

This is varied depending upon each person, but generally, we may gather the following:

  • Your full name (and that of your next of kin)
  • Your email address, and postal address
  • Your date of birth
  • The name of your workplace
  • Your medical/health history
  • Information about your healthcare insurance (so we can arrange payment)

We will never pass this information to anyone else unless we ask your permission first.

What we do with your personal information

We use your personal information with the intention of providing you with the best possible care.

  • We use your personal information as part of the therapy records that we are legally obliged to keep. These may either be written notes or held electronically.
  • We may also request that we anonymise your feedback and use it as a ‘testimonial’, which would feature on our website. We would only ever do this with your permission, and you can always change your mind later, and request that we remove any feedback you provide from our website.
  • We will never sell or release your personal information to any ‘third parties’ (such as other companies, or marketing agencies). We may share your personal information with your GP (or in urgent circumstances, a family member) or other medical or social care professionals. We would always request your permission to do this first. The only situation in which we would share this information without your consent is if we are required to by law.

How we store your personal information

  • We hold personal information digitally in a password-protected form on secure servers.
  • Any written information and materials are locked in a secure location when not in use in a therapy session.
  • Personal information is normally kept for 5 years. This is in case we need to use it again in the future (such as if you come back for an assessment or therapy). However, you can request that your personal information is deleted sooner.

Your Rights Regarding the Information Stored About You

You have the following rights with regard to your data that we hold about you:

  • The right to be informed – being told what data you hold about them and what you do with it.
  • To request a copy of your data that we hold.
  • The right to have any inaccurate data about you corrected.
  • The right to ask us to delete or destroy your data.
  • The right to limit the amount or type of data we use.
  • The right to request we move your data electronically to another business.
  • The right to object to us using your data.

Requests can be made over the phone, by email, face to face or in writing by letter.
